Case management is a program that
enables consumers with serious on-going mental or emotional disorders to live
safely and successfully in the community by ensuring that resources and
services are obtained when needed. Consumers are visited in their homes by case
managers on a
regular basis, referred and linked to agencies and resources, provided
monitoring and follow-up, and afforded transportation assistance on an as needed
basis. The level or intensity of Case Management provided to a given consumer is
determined by the consumer's current needs.
Case
managers advocate for seriously mentally ill consumers and, in many
circumstances, may serve as the backbone of the consumer's support
system. A principal goal for the Case Management program is to provide the types
and degree of support for consumers that will prevent or eliminate the need for
hospitalization. Case Management services are offered to both children and
adults, including those who are developmentally disabled. Intensive Case
Management services are also available for consumers who's mental health
needs are complex or particularly demanding of concentrated focus.
